急性上消化道出血是指屈氏韌帶以上的消化道疾病引起的出血,為臨床常見急癥之一,主要表現(xiàn)為黑便、嘔血、頭昏、心悸等,大量出血還可導(dǎo)致失血性休克而危及生命。出血不僅導(dǎo)致病人的生理變化,還對病人的心理產(chǎn)生巨大的影響。上消化道出血由于發(fā)病急,癥狀變化快,病情較危重,患者突然面對疾病威脅時(shí),會產(chǎn)生生理、心理、情緒等方面的變化,這些負(fù)性反應(yīng)又會加重疾病的進(jìn)程及病情,影響疾病的治療[1]。我科對2011年3月~2012年12月收治的138例上消化道出血患者隨機(jī)分成對照組66例和干預(yù)組72例,通過對72例患者綜合性心理干預(yù)后觀察患者前后出血量明顯減少,總有效率明顯增高?,F(xiàn)報(bào)道如下:
1 資料和方法
1.1 一般資料 選擇年齡在18~78歲,臨床癥狀為“嘔血和(或)黑便”的上消化道出血患者,由胃鏡檢查為活動性胃潰瘍或十二指腸潰瘍出血,以及糜爛出血性胃炎"同時(shí)排除惡性腫瘤引起的出血、嚴(yán)重心肺肝腎功能不全者、妊娠和哺乳婦女、食管胃底靜脈曲張破裂出血或藥物過敏者。將138例上消化道出血患者隨機(jī)分成2組,其中,對照組66例中,男35例,女31例,平均年齡(58.1±4.3)歲,胃潰瘍28 例,十二指腸潰瘍22 例,糜爛出血性胃炎16 例。干預(yù)組72例中,男 38 例,女 34 例;平均年齡(56.3±5.2)歲;胃潰瘍32例,十二指腸潰瘍 25例,糜爛出血性胃炎 15 例。兩組性別、年齡、病種等資料經(jīng)統(tǒng)計(jì)學(xué)處理均無顯著性差異(p>0.05),具有可比性。
1.2 方法
1.2.1 制訂護(hù)理干預(yù)方案 病人入院后均給予及時(shí)有效的治療。對干預(yù)組患者根據(jù)病人臨床表現(xiàn)、診斷結(jié)果、既往病史、性格特征、發(fā)病誘因、生活習(xí)慣、恢復(fù)狀況,家庭狀況等做出綜合評估,結(jié)合病人具體情況,對病人心理特點(diǎn)分析,得出以下幾點(diǎn)心理狀態(tài)并有針對性的給予綜合性心理護(hù)理:
恐懼型表現(xiàn)為驚慌失措,恐懼不安,多疑煩躁,情緒不穩(wěn),反復(fù)詢問病情。此類患者特別是初次出血者,看到自己大量嘔血或便血,直接感官刺激極易產(chǎn)生恐懼心理,對疾病缺乏認(rèn)識,擔(dān)心病情較重,預(yù)后不良,救治無望 [2]。護(hù)理過程中,要對患者進(jìn)行耐心的病情解釋和心理疏導(dǎo),要保持鎮(zhèn)靜不驚慌,以和平的心態(tài)、良好的情緒去影響患者,及時(shí)清理嘔吐物和排泄物,保持室內(nèi)干凈、整潔、有序,操作處置要穩(wěn)、準(zhǔn)、快[3],忙而不亂,讓患者對護(hù)士有信賴感,使其放松緊張情緒,感到安全舒適。
抑郁型表現(xiàn)為情緒低落,沉默寡言,對外界事物不感興趣,沮喪、悶悶不樂或獨(dú)自落淚、厭世,甚至絕望、拒絕治療等。此類病人多為多次出血、療效不佳、經(jīng)濟(jì)不濟(jì),體質(zhì)虛弱,加上某些操作帶來的痛苦,使病人長期處于痛苦絕望之中。對這類病人護(hù)理人員應(yīng)及時(shí)掌握心理變化,多與家屬溝通,詳細(xì)解釋每天的治療護(hù)理措施和計(jì)費(fèi)清單,告知新農(nóng)合或醫(yī)保優(yōu)惠辦法、報(bào)銷比例以及也可先治療后交費(fèi),減輕心理上的負(fù)擔(dān),同時(shí)告知不良情緒是疾病發(fā)生和加重的誘因,使患者保持舒暢愉悅情緒。在實(shí)施各項(xiàng)操作時(shí)要做到慌而不亂、急而不匆,適時(shí)實(shí)事求是地使用積極鼓勵(lì)性語言,并適當(dāng)?shù)亟榻B治療成功的案例,請類似案例現(xiàn)身說法。
焦慮型表現(xiàn)為煩躁不安,心神不寧,敏感多疑、睡眠不佳,心跳加快,血壓升高、食欲不振。常因病情來得急,對相關(guān)治療不予理解,毫無思想準(zhǔn)備,對疾病的擔(dān)心與挽救生命的矛盾而產(chǎn)生了嚴(yán)重的心理沖突。護(hù)理人員要把患者急于了解的病情、檢查結(jié)果、治療方案、護(hù)理措施及預(yù)后、飲食等方面的內(nèi)容告訴患者,使其對所患疾病有所認(rèn)識和了解,通過癥狀改善事實(shí)讓患者看到疾病治愈的希望,保持樂觀情緒,利于配合治療。并與患者多接觸多交談,多溝通,做好基礎(chǔ)護(hù)理,生活護(hù)理,做到體貼入微,同時(shí)讓家屬陪護(hù)在身邊,精心照顧,做好安慰,使患者放松心情,安心治療。
輕視型表現(xiàn)為癥狀不明顯、出血程度不重、自感平素體健,病情不重,有盲目樂觀的僥幸心理,不遵醫(yī)囑,不配合治療,不按要求進(jìn)食或過早活動或急于出院。對這類病人護(hù)士要告知患者疾病發(fā)生發(fā)展規(guī)律及飲食休息對疾病轉(zhuǎn)歸的重要性,囑其禁煙酒及咖啡、濃茶,忌食刺激性、粗硬食物,忌暴飲暴食,幫患者建立科學(xué)飲食和健康的生活方式[2]。
1.2.2 臨床效果評價(jià): ①痊愈: 臨床癥狀消失,出血停止,休克糾正,大便潛血陰性;②有效: 臨床狀和出血有不同程度改善,休克糾正,大便潛血陽性; ③無效: 臨床癥狀和出血無變化或加重。痊愈率加有效率為總有效率。
1.3 統(tǒng)計(jì)學(xué)分析 采用SPSS 13.0統(tǒng)計(jì)軟件進(jìn)行統(tǒng)計(jì)分析,采用軼和檢驗(yàn), P

上消化道出血因發(fā)病急,病情重,病因不明,視覺上的刺激,病情反復(fù)造成沉重的經(jīng)濟(jì)負(fù)擔(dān)等,患者多表現(xiàn)焦慮、恐懼、抑郁,灰心失望,甚至絕望,那么加強(qiáng)心理護(hù)理,積極對患者的消極心理進(jìn)行干預(yù),使患者樹立戰(zhàn)勝疾病的信心就顯得尤為重要。而一旦發(fā)生上消化道出血病狀,心理和情緒就會出現(xiàn)障礙及變化,直接影響疾病的發(fā)展與轉(zhuǎn)歸。故護(hù)理人員應(yīng)及早、主動地進(jìn)行心理分析,及時(shí)采取相應(yīng)的護(hù)理對策。而患者也希望在護(hù)患溝通中獲得與健康相關(guān)的知識以配合治療,從護(hù)患溝通中得到心理上的安慰[4]。在藥物治療同時(shí)給予心理疏導(dǎo)、護(hù)患溝通、健康宣教,使患者處于良好的心理狀態(tài),心理護(hù)理工作應(yīng)以權(quán)威性的解說答疑解惑,巧妙積極的語言和暗示疏導(dǎo)、寬容和撫慰病人,引導(dǎo)重建人生觀,對不同的病人采取相應(yīng)的心理護(hù)理措施。這樣在搶救治療及護(hù)理康復(fù)過程中,患者就能表現(xiàn)出良好的心理素質(zhì),并能主動配合治療與護(hù)理的全過程,使病情恢復(fù)加快,提高治愈率。

無創(chuàng)機(jī)械通氣是急性左心衰患者的有效輔助治療方法之一,可明顯改善患者的呼吸狀況,保證有效通氣,可明顯緩解患者的急性左心衰狀況,改善心肌功能,進(jìn)而明顯提高患者的生活質(zhì)量,改善患者的預(yù)后[1-2]。為了探討護(hù)理干預(yù)在急性左心衰患者使用無創(chuàng)機(jī)械通氣治療中的影響,本院選取2011年11月~2013年11月診治的急性左心衰患者142例,均行無創(chuàng)機(jī)械通氣治療,根據(jù)護(hù)理方案分為兩組,行常規(guī)護(hù)理和護(hù)理干預(yù),針對兩組患者的主訴恐懼情況、主訴配合情況、主訴并發(fā)癥情況進(jìn)行對比分析,現(xiàn)報(bào)告如下。

本院2011年11月~2013年11月診治的急性左心衰患者142例,年齡為47~81歲,平均年齡為67.3±5.9歲,其中男性108例,女性34例?;颊呔霞毙宰笮乃サ脑\斷標(biāo)準(zhǔn),且經(jīng)臨床表現(xiàn)和實(shí)驗(yàn)室檢查確診,排除患有其他器質(zhì)性病變、血液病、免疫性疾病的患者,均行無創(chuàng)機(jī)械通氣治療。根據(jù)護(hù)理方案分為兩組,常規(guī)護(hù)理患者71例為對照組,護(hù)理干預(yù)患者71例為觀察組,兩組患者的基礎(chǔ)資料(性別、年齡等)差異均無統(tǒng)計(jì)學(xué)意義(p>0.05),具有可比性。
1.2方法
觀察組患者實(shí)施護(hù)理干預(yù):⑴基礎(chǔ)干預(yù)。護(hù)理人員采用和藹可親的語言和患者進(jìn)行交流,建立良好的醫(yī)患關(guān)系,取得患者的信任,通過溝通與交流,幫助患者消除焦慮、抑郁、恐懼、害怕、緊張、擔(dān)心等不良心理因素,幫助患者建立配合治療的信心和決心,建立良好的干預(yù)基礎(chǔ)。⑵情緒干預(yù)。向患者講解疾病的相關(guān)知識和治療的有效方法,使患者了解到疾病治療的目的和意義,使其可以正視疾病的治療,在治療過程中,患者出現(xiàn)恐懼感時(shí)要給予及時(shí)的安慰和鼓勵(lì),采用支持性語言和肢體語言穩(wěn)定患者的情緒,提高患者的耐受性,消除患者的不安狀況,減輕患者的恐懼病癥。⑶行為干預(yù)。指導(dǎo)患者在治療過程中采用端坐位,將頭部與肩頸處于同一水平位置,將頭后仰,確保呼吸道通暢,佩戴合適的面部口鼻罩,調(diào)節(jié)松緊度,設(shè)定吸氣相壓力由6cmH2O開始,設(shè)定呼氣相壓力由2cmH2O開始,增到患者的耐受,要確保患者的面部皮膚無受損,并在皮膚接觸部位放置紗布,減輕患者的壓迫感,在皮膚接觸部位涂抹凡士林,3h更換1次,每次20min,給予患者局部皮膚按摩等。⑷病情監(jiān)護(hù)。注意觀察患者的呼吸節(jié)律和心率狀況,確保主動呼吸和呼吸機(jī)同步,注意保持患者的呼吸道通暢,在加濕器內(nèi)添加蒸餾水,將氣體濕化后進(jìn)入氣道,指導(dǎo)患者掌握正確的呼吸方法。對照組患者實(shí)施常規(guī)護(hù)理?;颊咴谥委熯^程中,給予患者常規(guī)的基礎(chǔ)護(hù)理,保證患者的病房通風(fēng)良好和環(huán)境的整潔衛(wèi)生,及時(shí)更換患者的床單被褥,只要患者實(shí)施正確的飲食和作息等。
1.3評定標(biāo)準(zhǔn)
主訴恐懼情況的評定標(biāo)準(zhǔn):⑴輕度?;颊卟换乇苤委?,無明顯的恐懼表現(xiàn)。⑵中度?;颊哂锌謶直憩F(xiàn),曾試圖回避治療。⑶重度?;颊哂袕?qiáng)烈的恐懼表現(xiàn),盡力回避治療,在護(hù)士幫助下才能完成治療。
主訴配合情況的評定標(biāo)準(zhǔn):⑴積極?;颊呖梢苑e極主動配合治療。⑵一般。患者可以基本配合治療。⑶較差。患者需要在護(hù)士幫助下勉強(qiáng)完成治療。
主訴并發(fā)癥情況的評定標(biāo)準(zhǔn):⑴輕度。患者面部受壓處皮膚發(fā)紅和疼痛,停機(jī)5~10min后可恢復(fù)正常。⑵中度?;颊咧髟V有腹脹感,可耐受,面部受壓處皮膚發(fā)紅和疼痛,停機(jī)5~10min不能恢復(fù)正常。⑶重度?;颊咧髟V腹脹明顯,行胃腸減壓和肛管排氣后恢復(fù),面部受壓處皮膚破損。

臨床上,輪狀病毒感染性腹瀉在兒科中具有較高的發(fā)病率,引發(fā)本病的病原體主要為輪狀病毒,此病的主要臨床癥狀表現(xiàn)除了腹瀉之外同時(shí)普遍并發(fā)有脫水、發(fā)熱等癥狀,嚴(yán)重的甚至出現(xiàn)代謝紊亂以及酸中毒,通常好發(fā)于秋冬季節(jié),若無法及時(shí)診斷并接受針對性治療,嚴(yán)重的甚至可直接引發(fā)死亡[1]?,F(xiàn)階段,臨床上對本病的發(fā)病機(jī)制尚無法明確,因而無統(tǒng)一的且具有明顯效果的治療標(biāo)準(zhǔn)[2]。為此,我院引入西咪替丁與干擾素聯(lián)合微量元素鋅治療,現(xiàn)將療效報(bào)道如下。
1 資料與方法
1.1 一般資料
67例均為我院自2015年11月~2016年12月期間收治的輪狀病毒感染性腹瀉患兒,納入標(biāo)準(zhǔn):臨床癥狀均符合本病的癥狀表現(xiàn),同時(shí)經(jīng)病原學(xué)檢查確診。經(jīng)醫(yī)院倫理委員會審批后,將67例患兒隨機(jī)分為治療組與對照組兩組,對照組32例,其中男17例,女15例;年齡0.7~4.5歲,平均年齡(1.6±0.8)歲。治療組35例,其中男19例,女16例;年齡0.8~4.4歲,平均年齡(1.6±0.7)歲。兩組患兒在基線資料方面對比差異無統(tǒng)計(jì)學(xué)意義(P>0.05),具有可比性。
1.2 方法
兩組均給予常規(guī)檢查,同時(shí)結(jié)合脫水的嚴(yán)重程度給予補(bǔ)液治療,給予發(fā)熱以及嘔吐者對癥治療,注意兩組均不能給予抗生素治療?;诖耍瑢φ战M采用西咪替丁注射液(湖南漢森制藥股份有限公司;H43021799)10~15mg/(kg?d)治療,給藥方式為靜脈滴注,同時(shí)增加重組人干擾素α1b [安徽安科生物工程(集團(tuán))股份有限公司;S20040010]治療,給藥方式為注射,劑量為100萬U/次,1次/d。在此基礎(chǔ)上,治療組聯(lián)合葡萄糖酸鋅口服液(湖北福人金身藥業(yè)有限公司;H20065022)治療,給藥方式為口服,每支的含鋅量為0.03g,1支/d,兩組的療程均為10d。
1.3 觀察指標(biāo)及評定標(biāo)準(zhǔn)
記錄兩組大便及體溫恢復(fù)時(shí)間、脫水糾正時(shí)間。記錄兩組面色潮紅、煩躁以及腹瀉加重等不良反應(yīng)情況。對兩組治療效果進(jìn)行評估,若3d后,癥狀消失,同時(shí)糞便次數(shù)以及形狀均正常則評定為顯效;若3d后癥狀明顯緩解,同時(shí)糞便次數(shù)及形狀明顯好轉(zhuǎn)則評定為有效 ;若3d后癥狀未見任何緩解,同時(shí)糞便次數(shù)及形狀未見任何改善則評定為無效??傆行?100%-無效率。
1.4 統(tǒng)計(jì)學(xué)分析
使用SPSS19.0統(tǒng)計(jì)學(xué)軟件處理數(shù)據(jù),兩組計(jì)數(shù)資料對比行χ2檢驗(yàn),兩組計(jì)量資料對比行t檢驗(yàn),用P
2 結(jié)果
2.1 兩組治療基本情況比較
治療組脫水、大便以及體溫等的恢復(fù)時(shí)間均分別明顯短于對照組(P
2.2 兩組治療效果比較
治療組治療總有效率為97.14%,明顯高于對照組的71.88%(P
2.3 兩組不良反應(yīng)比較
治療組不良反應(yīng)發(fā)生率為2.86%,與對照組的不良反應(yīng)發(fā)生率6.26%對比差異無統(tǒng)計(jì)學(xué)意義(P>0.05),詳見表3。
3 討論
臨床上,感染性腹瀉屬于兒科中常見的一種腸道感染性疾病,出現(xiàn)腹部疼痛以及糞便不成形等為本病的主要癥狀表現(xiàn)[3-4]。人體小腸內(nèi)部的載膜絨毛上皮細(xì)胞中存在一種可以與輪狀病毒外殼中含有的VP4相互結(jié)合的病毒原體,因此小腸成為了輪狀病毒侵入的重要部位[5]。待病毒繼續(xù)侵入機(jī)體上皮細(xì)胞之后,便會誘發(fā)機(jī)體內(nèi)的大量非結(jié)構(gòu)蛋白糖基化,進(jìn)而引發(fā)細(xì)胞膜發(fā)生紊亂現(xiàn)象,例如鈣離子通透性,一旦機(jī)體細(xì)胞內(nèi)的鈣離子出現(xiàn)大量增多的現(xiàn)象,便會引發(fā)高鈣低氯癥狀,進(jìn)一步可增加病毒非結(jié)構(gòu)蛋白NSP3抑制蛋白正常合成的風(fēng)險(xiǎn),進(jìn)而將影響小腸上皮細(xì)胞的運(yùn)行,嚴(yán)重的甚至可導(dǎo)致其脫落,極大的減少了雙糖轉(zhuǎn)化為單糖過程中所需的腸載膜上絨毛酶的數(shù)量,隨著時(shí)間的延長,雙糖大量堆積在腸道內(nèi),便極易增加腸腔內(nèi)滲透壓,進(jìn)而誘發(fā)腹瀉、嘔吐及脫水等癥狀,嚴(yán)重的可并發(fā)水電解質(zhì)紊亂以及酸中毒等嚴(yán)重威脅患兒生命的癥狀[6-7]。
西咪替丁是一種H2受體拮抗劑,能夠顯著抑制因組胺或者胃泌素引發(fā)的胃酸分泌,進(jìn)而避免受損腸黏膜長期處在酸性環(huán)境中而對其起到保護(hù)作用,并有利于為絨毛的修復(fù)創(chuàng)造良好條件[8-9]。此外,此種藥物還具有免疫調(diào)節(jié)作用,能夠通過提高患兒機(jī)體內(nèi)的T淋巴細(xì)胞的轉(zhuǎn)化率而起到增強(qiáng)其免疫力的效果,進(jìn)而能夠避免炎癥反應(yīng)誘發(fā)的一系列繼發(fā)性損傷問題[10-11]。干擾素則具有免疫調(diào)節(jié)以及抗病毒效應(yīng)等多種生理功能,在本病的治療過程中具有顯著的價(jià)值[12]。刷狀緣中大部分酶類均對鋅元素存在較強(qiáng)的依賴性,同時(shí)鋅離子能夠顯著的抑制小腸黏膜的增生,且能夠促進(jìn)其完全角化,進(jìn)而能夠?qū)δc黏膜起到保護(hù)作用,此外,還可抑制胃腸激素分泌,增加腸內(nèi)鈉氯離子的吸收能力,進(jìn)而能夠?qū)δc腔的滲透壓起到降低的效果[13]。除此之外,鋅離子還具有免疫調(diào)節(jié)以及抑菌等多種功效。特別值得關(guān)注的是,我院在采用西咪替丁與干擾素常規(guī)治療的基礎(chǔ)上增加微量元素鋅的一個(gè)理論基礎(chǔ)是此類型的患兒血清中鋅代謝有異常的現(xiàn)象[14-16]。
本研究中,治療組脫水、大便以及體溫等的的恢復(fù)時(shí)間均分別明顯短于對照組(P
綜上,將西咪替丁與干擾素聯(lián)合微量元素鋅應(yīng)用于輪狀病毒感染性腹瀉患兒的治療中,可以快速消除患兒的癥狀,促進(jìn)其康復(fù),具有療效理想,安全性高的特點(diǎn),值得臨床上推廣及應(yīng)用。
